The intake forms on this Site are provided and hosted by our HIPAA-compliant intake platform. When you submit an intake, your answers are transmitted directly to that platform and to the clinic’s clinical team. The Site itself does not store, read, or forward your intake answers.

  • Contact you initiate. If you email or call us, we receive the information you choose to share.
